Over a decade ago, high quality healthcare has been a goal of countries around the world.
Despite many efforts to improve the patient’s health, most continue to receive standard care and avoidable harm.
As a registered nurses, it is my duty ensure that patients are safe at all time.
High quality of care is vital as it helps nurses protect patients from harm.
Since patients can live a healthy lifestyle, I aim to improve patient safety.
Patient safety depends on having the right knowledge and training.
The Reasons for Patient Safety
It is vital to instill a sense that urgency as patients need high-quality healthcare to improve their quality life.

Additionally, leaders and managers often make fatal mistakes like declaring victory. This can cause low momentum when trying to improve patient safety.
It is important for individuals to understand and identify the stages of transformation and the pitfalls. This will increase conversion rates (Kotter 2007).
The communication of a vision and the creation of strategies for implementing it helps to teach new behaviors and help me achieve my goal of increasing patient safety ( West Lyubovnikova and Eckert, 2014, pp. 240-260).
Empowering others in the pursuit of this vision encourages taking risks and encouraging people to do the same.
Patient safety is a choice I made because every person has the right and obligation to receive quality healthcare services.
There are many patient safety methods that can help avoid safety issues and improve patient care. These include crew resource management and the use of stimulators.
There are many errors that could occur, regardless of the expertise or environment.
The possible causes can be identified as follows: A potential failure is when doctors are not involved with decisions that affect the organization’s policies and allocation of resource (Weberg 2012). 268-277).
Active failure involves direct contact with patients.
Additionally, an organizational system breakdown can result in indirect failures that impact the management, corporate cultures, procedures, knowledge transfers, and other procedures.
It is important to support safety culture in order to avoid harm and errors (Swartling und Poksinska 2013, pp. 81-94).
The culture of safety’s core beliefs and actions demonstrate that there is a commitment not only to patient safety but also to other objectives.
In order to enhance the patient’s safety, it is essential that there is good communication between the patient and their family.
I will make sure to meet the nurse leader in order to get the problem resolved early enough to offer proper assistance. (Serrano 2010, P. 1-4).
Also, basic care is vital. It’s important to follow the checklists for things such as turning the patient and giving oral care.
In order to help patients open up about their problems and to discuss their goals, it is vital to ask them questions and engage them in conversation about the care they are receiving.
This will allow patients to speak openly with the nurses and share all their experiences.
This will help improve the relationship between the nurse and patient in the healthcare facility.
The hospital management must ensure that there are democratic powers to ensure that all practitioners work together for patient safety.
This will encourage nurses to excel as they will put patient safety first.
Alternatives, Desired state and Evidence
Nursing has always been concerned about patient safety, quality improvement and safety in healthcare.
In the past, nurses were viewed as being responsible in some way for the safety and well-being of patients, such as avoiding any medical errors or preventing falls (Luxford Safran and Delbanco (2011), pp. 510-515 ).
It is important to remember that patient safety must be considered in all aspects of nursing practice.
Thom et.al. (2016) state that nursing’s critical contribution to patient safety is its ability coordinate and integrate all aspects of quality in nursing care. 2016, pp.819-822 ).
Positive culture in nursing is one of empathy, support, transparency, respect and mutual respect. This culture is non-hierarchical.
The value of relationships between nurses and patients is evident. This shows that there is a culture where improvement can be achieved.
Nurses have to work together to accomplish team goals and must be part of decision making.
They are caring and put patients first ( Baker, Salas Battles, King 2016 pp. 212-225 ).
They are also able to support the health and well-being of patients by recognizing their emotional, spiritual, and physical needs.
Nurses make sure patients are safe by making sure they have time to train and develop.
Dipboye (2016) states that nurses should constantly use evidence and data to improve (Dipboye, 2016).
The nurses must have a positive attitude to be able to take full responsibility for their hospital, services and reputation.
They are encouraged encourage to be open to challenging others and respond positively to being questioned (Jex. 2014).
This is vital for their patients as it will make them feel safe and allow them to respect the nurses’ decisions concerning their health.
Factors to Consider in Addressing the Problem
In order to ensure patient safety, the leadership style is crucial.
The hospital’s management should ensure that all doctors and nurses are aware of the hospital’s vision and mission to ensure patient safety (Braithwaite Wears, Hollnagel 2015).
Professional domain refers to attributes like meaning, autonomy, purpose and values.
Professional loyalty is also placed before organizational improvement. In other words, professional autonomy is tied to loyalty to the patients (Taylor Parekh. Estrada. Schleyer. Sharpe 2014. pp.214-218).
Engaging clinicians is necessary to ensure that there is an alternative paradigm of the dominant clinician/manager.
This will enable them both to work together in ensuring patient safety.
Healthcare must find their purpose and remain loyal to their patients.

It is important that the conditions in which health professionals work are conducive to providing quality care for patients.
A well-structured team is also important as it helps to maintain the safety of the patient (Singh, Sittig 2016, pages 226-232).
They work together in finding solutions to current safety threats.
Transparency and honesty are the keys to patient safety (Vaismoradi Jordan & Kangasniemi 2015pp.627-63)
So that patients can receive the care they need, healthcare professionals must be open-minded.
It is important to provide quality care as it helps nurses protect patients.
Management and leadership are key to solving safety problems for patients.
